Diagnostic Criteria of Erection Penogram with Papaverine and 99mTechnetium-Human Serum Albumin

Abstract
The diagnostic method using radioisotope for impotence has been introduced in 1970. Since then, several other investigator tried to develop the radioisotope study for impotence. Recently "Erection Penogram" has introduced as a new screening method to estimate the penile flow changes during artificial erection with intracorporeal papaverine injection and 99m Technetium-Human Serum Albumin via antecubital vein simultaneously, in order to calculate the radioactivity of the penis using computerized gamma camera. This method must be attractive in differentiation between arteriogenic and venogenic cause. But its diagnostic criteria was so subjective, and therefore more acceptable criteria would be needed. The purpose of this study is to evaluate erection penogram in 5 normal volunteers who had undergone AVS penogram, 5 arteriogenic impotence confirmed selective internal pudendal arteriography and 4 venogenic impotence who experienced striking improvement of erection after the ligation of deep dorsal vein. Using the above mentioned study, even the number of patient population is small, new diagnostic criterias in erection penogram are proposed. The criterias are,
1. Normal person

Erection angle-single phase

Transit time-less than 300 seconds

Erection persistent time-more 1000 seconds

Venous index-about 1.00

2. Arteriogenic impotence

Erection angle-multiple phase

Transit time-more than 600 seconds

Delayed transit time (more than 600 seconds) is a definite diagnostic criteria.

3. Venogenic impotence

Erection persistent time-less than 500 seconds

Venous index-less than 0.80

In conclusion this erection penogram method has several advantages in evaluating the impotence but other unanswered questions should be addressed in the future.
